Cluster headache

Cluster headache is far less common than migraine headache or tension headache. Cluster headaches begin far more dramatically, however, and remain quite unique in their course over time.
As the name suggests, the cluster headache exhibits a clustering of painful attacks over a period of many weeks. The pain of a cluster headache peaks in about 5 minutes and may last for an hour. Someone with a cluster headache may get several headaches a day for weeks at a time - perhaps months - usually interrupted by a pain-free period of variable length.
In contrast to people with migraine headache, perhaps 5-8 times as many men as women have cluster headache. Most people get their first cluster headache at age 25 years, although they may experience their first attacks in their teens to early 50s.
You can get 2 types of cluster headache:
  • Episodic: This type is more common. You may have 2 or 3 headaches a day for about 2 months and not experience another headache for a year. The pattern then will repeat itself.

  • Chronic: The chronic type behaves similarly but, unfortunately, you get no period of untreated sustained relief.

Cluster Headache Causes

No one knows exactly what causes cluster headaches. As with many other headache syndromes, theories abound, many of which center on your autonomic or "automatic" nervous system or your brain's hypothalamus. These systems play a role in rhythmic or cyclical functions in your body. The involvement of either system in the syndrome would account for the periodic nature of the headache.
  • Many experts believe that cluster headache and migraine headache share a common cause that begins in the nerve that carries sensation from your head to your brain (trigeminal nerve) and ends with the blood vessels that surround your brain.

  • Others believe that the pain arises in the deep vascular channels in your head (for example, the cavernous sinus) and does not involve the trigeminal system.

Cluster Headache Symptoms

The pain of cluster headache is its defining and most dramatic feature. This pain comes on without warning (no forewarning symptoms such as the aura in classic migraine) and may begin as a burning sensation on the side of your nose or deep in your eye.
The pain peaks in just a few minutes. People describe the feeling as having an ice pick driven through your eye. They use words such as "excruciating," "explosive," and "deep." This stabbing eye pain carries with it a rapid electrical-shocklike element, which may last for a few seconds, and a deeper element that continues for a half-hour or longer. The pain almost always begins in your eye and always on 1 side of your face. Interestingly, for most people the pain stays on the same side of the face from cluster to cluster, while in a small minority the pain switches to the opposite side during the next cluster.
In addition to its one-sidedness, other characteristics separate cluster headaches from other headaches.
  • The headaches commonly come on just after you go to sleep.

  • Often the eye on your affected side will tear.

  • Your eyelid on the affected side will droop.

  • You will experience one-sided nasal stuffiness and runny nose

Spain comes to final

First Europe and now, possibly, the world for Spain, who won through to the Final of South Africa 2010 with a 1-0 victory over Germany in Durban. Carles Puyol's 74th-minute header repeated the single-goal triumph over Germany that secured La Roja the European title two years ago and now only the Netherlands stand between them and a first FIFA World Cup™ title.
Whatever the outcome at Soccer City on Sunday, there will be history made with a new name on the Trophy after Spain produced their best performance of these finals to end Germany's hopes of an eighth Final appearance and secure their first. Joachim Low’s men, by contrast, missing the suspended Thomas Muller, were unable to repeat the scintillating displays with which they swept aside England and Argentina and, as in 2006, suffered the anguish of semi-final defeat.
While Germany were playing in their 12th FIFA World Cup semi-final, this was Spain's first, although it was business as usual for Vicente del Bosque's side, who dominated possession. Indeed Spain might have had an early goal when Pedro, making his first start of the finals in place of Fernando Torres, slipped a through-ball to David Villa after just six minutes. Clear of the Germany defence, Villa produced a sliding finish but Manuel Neuer was out of his goal fast to deny the Spaniard.
There was another nervy moment to follow for Low's men after 14 minutes. From a short corner, Andres Iniesta drove in a centre that Puyol met with a flying header that, to the relief of the Germans, cleared the crossbar. Spain had more than 60 per cent of the ball in the game's first quarter but Germany, happy to sit deep and continue the counter-attacking game that had brought them such reward in previous matches, began to offer a threat. Lukas Podolski played in Mezut Ozil on the left and he duly supplied Miroslav Klose on the edge of the box, but the Bayern Munich forward was crowded out.
Iker Casillas was called into action for the first time just after the half-hour to turn behind a low 30-yard drive from Piotr Trochowski, the man brought in to replace Muller. On the stroke of half-time, Germany finally picked a hole in the Spain defence when Ozil broke into the box on to a pass from Klose. As Sergio Ramos challenged, the German midfielder went to ground but referee Viktor Kassai waved play on.
The second half began like the first, with Spain threatening Neuer's goal as Xabi Alonso drove narrowly wide from 25 yards, then Villa curled another attempt wide of the same post. The pressure intensified with the hour approaching and Germany's goal was lucky to survive intact. Pedro's low shot drew a fingertip save from Neuer and as Per Mertesacker dawdled over his clearance, Iniesta nipped in and drove a low ball across goal that the lunging Villa was within a whisker of reaching at the far post. With Germany still unable to clear their lines, Pedro then fired wide.
Low sought to change things, sending on Marcell Jansen in place of Jerome Boateng and, later, Toni Kroos for Trochowski. It was Kroos who had Germany's first attempt of the second half in the 69th minute, meeting Podoski's far-post cross with a side-footed shot that Casillas beat away.
Instead, with 16 minutes remaining, the decisive goal came at the other end. From a corner by Xavi, Puyol leapt above team-mate Gerard Pique and powered a header past Neuer. Pedro could have ensured a bigger margin of victory in the closing stages but he allowed Arne Friedrich to dispossess him after bursting through in a two-on-one with substitute Torres. It did not matter in the end, Spain's third successive 1-0 win carrying them into their first Final.
In the final, Spain will againts Netherlands, which send home Uruguay 3 - 2. So, which team will be the champion? just wait and enjoy the final match.

Congrat Rafa !!!!

Rafael Nadal made a simple job of winning his second Wimbledon men's crown, swatting aside the giant Czech Tomas Berdych 6-3, 7-5, 6-4 in two hours 13 minutes. Those who had feared that the 6ft 5in Berdych would be overwhelmed, but not overawed, were proved correct as Nadal never dropped serve in three routine sets, centring his winning attack on that formidable forehand, the best shot of its kind in tennis.

Nadal came into the final having won six straight matches against Berdych, accumulating 14 sets in doing so, and those statistics were extended to 17 sets with no discernible problem.

This is Nadal's eighth Grand Slam title and the second time he has come straight from winning the French Open at Roland Garros to lift the Wimbledon title.

A swirling breeze threatened to affect the timing of both of these big-hitters but initially, at least, both coped admirably with the wind and a bright sun, each holding serve to love as they tested out each other's reactions.

The Centre Court crowd was vociferously behind the Spaniard, with one (male) voice booming out "I love you, Rafa", soon followed by "You're a genius, Rafa" from the same section, a fact which the world No.1 underlined with some fine, tight serving in the opening set, in which he conceded a paltry four points on serve.

Berdych stayed in the hunt with excellent serving and heavily-struck forehands until he faltered in the seventh game, offering Nadal a glimmer of opportunity by directing a backhand long to fall love-30 behind. Rafa pounced in a flash with a great forehand pass to open up break points and captured the Czech serve with a backhand service return with which Berdych could not cope.

Now in full, exciting flow Nadal made it four games in a row by breaking Berdych again to take the first set in 34 minutes. Then, at the start of the second, he made it five games in succession, albeit with some difficulty in a service game which lasted 11 minutes and in which the Czech held three break points without managing to capitalise on any, despite Nadal's best effort to assist him with a pair of double-faults.

After that marathon game, with Berdych shaking his head in despair at the chances he had missed, normality was restored on serve, both men holding with confidence and comfort. Aware that Berdych urgently needed to get back into this final, the crowd were behind him every time he captured a point on the Nadal serve, something which did not happen frequently.

With Nadal leading 6-5 the second set seemed destined for resolution by tiebreak, since the Czech had permitted Nadal just four points in five service games. Then, spectacularly, he imploded, dropped his delivery to love with a forehand driven tamely wide and Nadal was two sets in front with an hour and 28 minutes gone.

Berdych's urgent need was to get into the Nadal serve, get a break somehow and get himself back into a match which was inexorably draining away from him. But the Spaniard was not in a "giving" mood, bearing down as he headed towards another triumph on the lawns of London.

In a rare moment of relaxation, Nadal permitted Berdych a break point in the third game of the third set but the Czech dumped a backhand into the net and the opening was lost. For the rest, the howitzer that is the Nadal forehand was running the show.

And it was that forehand which decided it in the tenth game as Berdych served to stay in contention. A couple of Nadal forehands had the Czech groggy at deuce and when he over hit his own forehand to offer Rafa his first championship point, it was all he needed, clinching his second Wimbledon with a stupendous cross-court forehand before falling flat on his back in the baseline dust and then executing a celebratory forward roll.

Serena wins Wimbledon 2010

It was all so easy for Serena Williams as she hunted down her fourth Wimbledon title and 13th Grand Slam. Just 67 minutes were all the 28-year-old American needed to swat aside the challenge of Russia's Vera Zvonareva 6-3, 6-2 on a sunlit Centre Court to retain the Ladies' Singles Championship.

The £1m prize money, the highest ever, will be a welcome aside for the world No.1 and top seed. What she was really after was yet another trophy for the Williams family at the world's leading tennis tournament and to kick off a new Wimbledon decade after the Noughties were dominated by her and older sister Venus. After today's victory, the title tally is Serena 4 Venus 5, but the 28-year-old younger sister is catching up fast.

Zvonareva, the 21st seed and appearing in her first Grand Slam final, paid full tribute afterwards. She told Serena, as the winner clutched the Venus Rosewater Dish: "You are a great player and also a great champion. You really deserved to win today." And so she did.

Williams opened with a flourish, holding serve to love and throwing in an ace timed at 114mph, and though Zvonareva's serve was matching the American's - for accuracy if not power - the ball was invariably coming back at her faster than she had dispatched it.

Trying for service power cost the Russian the occasional double-fault but she had better luck when driving the ball high to the baseline, particularly on Williams' backhand, rather than hitting flat and hard.

Williams' ferocity was made clear on her regular aces, which generally clocked in at around 115mph, and despite the loud support for "our Vera" from a crowd keen to see an equal match, it was Williams who started to press for the advantage. She had a break point in the sixth game as Zvonareva went for the lines and narrowly missed three times, but the break was saved with a brave forehand and Zvonareva held with an ace.

But that merely postponed the inevitable until Zvonareva stepped up to serve again at 3-4. Despite having two game points for 4-4, she was reeled in by the relentless Serena, who needed three break points to clock up a 5-3 lead but achieved it, brilliantly, at the third time of asking with a sensational forehand pass.

Throwing another ace into the mix, Williams served out for a one-set lead after 36 minutes, and while the Russian was still reeling Serena also poached the opening game of the second set to accelerate the Zvonareva slide. Now Williams was in full majestic, athletic flow and there is no-one else in the world of women's tennis capable of containing her in such a mood.

She broke again for a 4-1 lead, courtesy of a Zvonareva double-fault and the Russian's fate was virtually confirmed. In the last couple of games, the 25-year-old Muscovite looked in urgent need of body armour as Williams pounded down some huge shots. The final game started with Williams' fastest ace of the afternoon at 122mph. It was also her 89th ace of the fortnight, a record for any woman here at Wimbledon.

As if to underline her powerful grip on the game, the tournament and the world of women's tennis right now, she followed that up with a succession of thunderous overheads, the last of which sealed her fourth title.

At the moment of victory, the racket which had propelled the ball so fast and so accurately was hurled high into the air as Williams did a quick bow to the crowd and then raised prizefighter-style double clenched fists. The undefeated champion once again.

In 4-0 loss to Germany, Maradona's questionable tactics come to light

Earlier this week, on a practice field in Pretoria, Diego Maradona kicked the ball around with his players while puffing on a giant Cuban cigar. This is the guy who demanded a 24-hour ice cream bar at his team headquarters, whose unorthodox training method includes the game piggy-in-the-middle, in which the loser bends over and lets teammates fire shots at his rear end. His game tactics and personnel decisions were up for debate, but the 49-year-old icon had injected fun into the Argentine camp, infused players with his boundless energy, and it seemed to be working. Argentina was the highest scoring team in this World Cup heading into Saturday's quarterfinal against Germany, and with Brazil bowing out a day earlier, many experts had picked Maradona's merry bunch to win it all.
But for the second day in a row, European organization and precision beat South American flair in a game billed as a mouthwatering showdown between the tournament's two highest-scoring teams in the country's most scenic venue.
Maradona was quiet and disconsolate in the bowels of Green Point Stadium on Saturday night after a 4-0 thrashing at the hands of the Germans, the worst Argentine World Cup loss in 36 years. Thomas Mueller headed in the go-ahead goal in the third minute off a Bastian Schweinsteiger free kick, the quickest goal scored at this tournament, and quickest ever conceded by Argentina.
Germany would score twice more in a six-minute span — a 68th-minute strike by Miroslav Kose and 74th-minute shot by Arne Friedrich — and then Klose added the fourth in the 89th minute. Klose is tied in the Golden Boot race with four goals, and his 14 career World Cup goals ties Gerd Muller for second all-time, one shy of Ronaldo's record.
It was the second World Cup in a row in which the Germans denied the Argentines a spot in the semifinals. Germany, which has outscored opponents 13-2, now faces Spain, a 1-0 winner over Paraguay.
On this night, there would be no silly banter between Maradona and reporters, no outrageous comments.
Maradona, who gave his daughter a long hug on the sideline after tucking his rosary beads into the pocket of his gray suit, called Saturday's loss "the toughest moment of my life." That is saying something for a man who has battled a cocaine habit, alcoholism, weight gain, and tumultuous romances.
He talked about how tough it was to walk into the locker room and see Lionel Messi, the 2009 FIFA Player of the Year, in tears, heading home without scoring a single goal. "Anybody who says Messi has no feeling for the Argentine shirt is stupid."
Asked whether the loss compared with any other low points of his life, he said: "Well, the day I stopped playing football could have been similar, but this sadness is really strong, it's tough. It is tough because the wish to go beyond today, and to be among the four best teams in the world, we didn't achieve that. We all had this hope, this dream since we left Pretoria. We were just thinking about winning over Germany, and the opposite happened.
"I lived through this in '82 as a player. But I was a boy and I didn't really realize the importance of things. Today, I will be 50 years on October 30, and this is the toughest moment in my life-having so many good people, so many good professionals, so many great players-this is really like a kick in the face. I have no more energy for anything."
The players were equally distraught.
It took them nearly two hours to emerge from the dressing room. They trudged through the interview area, heads down, most of them refusing to talk to reporters. Only three players spoke — Carlos Tevez, Gabriel Heinze and captain Javier Mascherano.
"This is the toughest moment of my career," said Tevez, who displayed his usual tireless work rate, but was stuffed by Germany's defenders. "It was the dream of all of Argentina, and nobody's sadder than we are. We had a bad game, and we paid. I, personally, don't have any regrets. I leave here calm. I left everything I have on the field in every game."
Added Heinze: "Believe me, I am twice as sad as you (reporters) are. Nobody hurts more than we do. It's been years preparing for this, we all knew what we had to do. I will defend this team because I lived it, went through a lot, and in this 30 days we had a great group. (Saturday), we were not able to do it. In football sometimes these things happen. We were playing well, but everything went wrong. Once in my life I wanted to write the last headline, and now you guys will get to write it, and that hurts."
Meanwhile, German coach Joachim Lowe and his players celebrated and tried to explain how they so clinically took apart a team that had won all its matches convincingly. They suffocated Messi all night long, forcing him to drop back to get balls. And they used their speed and precision to counter-attack very effectively.
"We observed the Argentina team fairly carefully," Lowe said. "We knew Messi would fall back into the midfield. He is of course a key player and a pass-giver for the Argentina team, so we tried to keep him under control and tried to neutralize him by putting him under pressure or staying close to him."
He said they also took advantage of the fact that Argentina's attackers don't always support the defense, creating space, which an opponent can exploit if it acts quickly enough. And the young Germans did. Time and again.
"It's insane to beat Argentina 4-0," said Mueller, who has to miss the semifinal after being booked with a second yellow card. "It's hard to find the words for a result like that."

And from other quarterfinal match, Spain beats Paraguay 1- 0 to make a road to semifinal.

Rafa beats Murray in Wimbledon semifinal

Andy Murray's hopes of becoming the first British man for 72 years to reach a Wimbledon final were blown apart by an inspired Rafael Nadal, who won a riveting Centre Court battle 6-4, 7-6, 6-4.

Until the later stages of the final set, Murray can rarely have played better, particularly on serve, but Nadal is not world No.1 (and second seed here) without merit. In front of a packed crowd who were clearly rooting for his opponent, the man from Mallorca was magnificent and unbeatable. He will go into Sunday's final against Tomas Berdych as clear favourite to win again, as he did in 2008.

Clearly keyed up for this, one of the biggest contests of his career, Murray bellowed "Come on" after taking the opening point of the match with a service winner, and followed this with the first of five aces he was to deliver in the first set. Knowing that he had to perform at top level to stay with the world No.1, Murray was doing just that.

He was solid on his first serves, averaging 73% on target, and reliable on his second when the first one missed. The Scot's first four services games cost just four points but Nadal, the great predator, was not only very comfortable on his own delivery but lurking to strike the moment he spotted the slightest opening, which happened in the ninth game.

A stupendous service return forced Murray to strike his next shot short into mid-court and Nadal was on it in a flash, hooking a forehand winner to take him to break point, and then accepting a gifted break as Murray missed an easy forehand.

Nadal served out efficiently in the next game and the first set was his in 37 minutes. The depressing statistic for Murray was that there had been just one Nadal unforced error in the whole set.

All Murray could do in the second set was to keep serving at top level and hope for Nadal to start making mistakes. The Scot certainly did his part, not conceding a single point on serve until the seventh game of the set, and that was the result of a fluke bounce.

And suddenly, too, there were unforced errors from the Spaniard - three in the sixth game, from which he escaped, and more in his next service game, when, for the first time, Murray held a pair of break points. He could not cash in, but he was getting closer.

But not close enough to prevent the second set moving to a tie-break where, to mounting hysteria in the crowd, Murray served successive aces to put himself within reach of winning it. Nadal then double-faulted, leaving the Scot at set point, but he could not clinch it. Nadal swept the next three points, one fortunately with a net cord and then went two sets up when his howitzer forehand delivered another unreachable shot.

Defiantly, Murray opened the third set by breaking Nadal to love and then conceding just two points in his next three service games. But he needed to avoid dropping serve for just two more games when leading 4-2, and it proved beyond him as his first service percentage dropped right down to 40%. Murray never won another game in that third set as Rafa revved up to his most dynamic level.

In final, Rafa will meet Thomas Berdych. Berdych beats Novak Djokovic 6-3. 7-6 (11-9), 6-3 in the other semifinal.

Uruguay comes to Semifinal

Ghana became the final African team to say goodbye to the 2010 FIFA World Cup™, losing in a dramatic penalty shoot-out to Uruguay after a 1-1 quarter-final draw at Johannesburg's Soccer City. Goalkeeper Fernando Muslera saved efforts from Stephen Appiah and Dominic Adiyiah, before Sebastian Abreu converted the decisive spot-kick for the South Americans, who reach the last four for the first time since 1970.
The Black Stars will have to be content with having become just the third side from the continent to make the quarter-final stage, although they could have won the match with the final kick of extra time. However, their top scorer, Asamoah Gyan, smashed a penalty-kick off the bar that would have tipped the scales to the Africans after Sulley Muntari and Diego Forlan scored either side of half-time.
Uruguay started the match on the front foot, looking dangerous from dead-ball situations and through the dynamic duo of Forlan and Luis Suarez up front. Suarez had the first real chance in the 11th minute when he beat a Ghanaian defender down the the left. He cut into the box but blasted his shot right at goalkeeper Richard Kingson.
Kingson, who is based at Wigan Athletic in England but rarely plays at club level, was the hero for the west Africans twice in short succession. In the 18th minute, from Forlan's corner, he reacted quickly to keep out the ball after it deflected off John Mensah and was seemingly destined for the back of the net. Seven minutes later, he rescued another defender, this time Isaac Vorsah, who missed a tackle allowing Suarez in on goal. But the net-minder just got his fingertips to Suarez's effort to push it over acrobatically.
The Black Stars then settled into the match and only narrowly missed going ahead twice near the half-hour mark. From Ghana's first corner, Vorsah leaped well but headed just wide of the post to the goalkeeper's right. A minute later, Gyan skimmed the other post after a strong run and cross from Kevin Prince Boateng. The Portsmouth midfielder almost opened the scoring himself in spectacular fashion in the 45th minute, but his overhead kick in front of goal spun off his foot. However, the west Africans got their breakthrough on the stroke of half-time as Muntari scored with a curling 35-yard shot that seemed to deceive the goalkeeper before finding the left-hand corner of his goal.
But if they ended the first period on a high, the Black Stars started the second on a low as Forlan's free-kick from near the left corner of the box fooled Kingson with its swerve in the 55th minute and bulged the net. The Atletico Madrid hit-man continued to play with confidence and in the 63rd minute he brought the ball down well on the left side of the box in a dangerous position. His cross beat Kingson, but Suarez, who had three goals coming into the match, could only volley into the side-netting at the far post.
As the match opened up and both teams searched for a winner, Suarez had another good opportunity in the 71st minute when sprung free into the box after some good combination play involving Forlan. But from a tight angle on the left he could not beat Kingson, who punched over. Uruguay then went close twice from Forlan free-kicks from the right wing. First, he screamed in a shot that beat the keeper but went into the side-netting and next he found Suarez in the box, but the Ajax man's header was tipped over the bar. Maxi Pereira then had an opening but failed to even test Kingson in the 82nd minute after a quality Uruguayan counter-attack left him in space 25 yards out.
Ghana had the clearest opportunities in extra time. Gyan flashed an open header over the bar in the 110th minute, and five minutes later, defender Andres Scotti did just enough to scramble the ball away when it looked like the Ghanaian would score. Boateng almost won it minutes from time, but his header screamed agonisingly wide. And then at the death, from a free-kick into the box, Suarez blocked Appiah's shot on the line only to then stop Adiyiah's headed follow-up with his hand to set up Gyan's penalty miss.
With the gasps of a stunned Soccer City still ringing in the ears, Gyan bravely stepped up to immediately convert the first penalty in the shoot-out. Pereira missed his spot-kick for the Uruguayans, but two consecutive saves by Muslera set up Abreu to be the hero.

Netherlands send home Brazil !!

Hahahahaha, finally Netherlands booked a place in the semi-finals after the 2010 World Cup quarter-final match at the Nelson Mandela Bay Stadium, Port Elizabeth, defeating Brazil 2-1.

Success put myself in this big four at the same time pay a bitter results obtained in the Dutch World Cup 1998 in France. At that time, the Dutch eliminated after losing via shootout.

In that game, the Dutch took the initiative to attack. Dirk Kuyt to get the ball from Robin van Persie did not provide significant threat to Brazil's defense early in the game.

Brazil had scored first when the match just eight minutes to walk through Robinho after receiving feedback from Luis Fabiano. But referee Yuichi Nishimura goals disallowed, because the Samba team striker is currently off-side position.

But Robinho scored only limited delays. For two minutes later, Robinho who received feed back from Felipe Melo penetrate the Dutch goalkeeper, and this time not in the off-side position.

ECLIPSE the Twilight Saga

This part of the story starts with the note Jake sent to Bella. It speaks of him missing her and that it changes little. Charlie blames Edward for Bella’s disappearance in New Moon as she travelled to Italy and her apparent craziness. He no longer likes Edward and shows with every ounce of his being that the vampire is welcome in the home. Seattle is also hit by a string of unsolved killings. Bella and Edward spend some time filling in college application forms and she tells Edward her plans and need to see Jacob once again. Naturally, Edward is against it, telling her werewolves are random and unpredictable, hurting those around them.

In the meantime, Alice has a strange vision in which Bella is being hunted by a vampire. Edward hides this information from Bella and plans a trip to Florida with Charlie to see her mother out of town. Upon their return Edward is confronted by an angry Jacob, he demands to know why the return of Victoria is being kept secret to Bella and why they crossed into werewolf territory chasing Victoria. Bella is dumbfounded as Edward informs her that the trip away from Forks was for her own personal safety.

Once again, Edward insists Bellas banning from seeing Jacob is to keep her safe and away from the dangerous werewolf. Bella, unbelieving insists that the pack are quite safe and that she’s never in harm’s way around Jacob. Finally, Edward yields and travels to the border with Bella where they meet Jacob and travel on to his house. During one such visit Jacob pleads his love for her and forcefully kisses her against her will. Unable to resist his power she fights him off with all the strength she can muster. When he ceases the kiss she punches him as hard as she is able but only manages to break the bones in her delicate hands. Werewolves incidentally, are nearly as durable and tough as vampires.

When Bella returns home and he learns of this Edward threatens Jacob angrily. Jacob tells Edward what occurred and they finally settle on a deal to work in unison to deal with Victoria. Later on that evening when Jacob has left, Edward and Bella decide to marry but only on the one condition that is they sleep together before she is turned into a Vampire. Edward realizes the severity in Bella’s voice and eventually agrees to try in the future but only once they have married first. Bella speaks of her concerns of what people will say and think if she marries at such a young age, this is partly confounded by her mother’s life and the fact she too married at such a young age. Spending the rest of eternity with Edward is of the utmost importance to her though, and when Edward proposes she quickly accepts. In an effort to keep her reluctant marriage secret she refuses to place the engagement ring on her hand.

Bella soon realizes that her possessions have been stolen from her room to use for scent and locating purposes. Also, it becomes apparent that the recent killings in Seattle are being perpetrated by a coven of young vampires. Bella realizes the connection and the fact Victoria has been leading these newborns. Alice predicts an epic battle between the combined force of Cullens and Werewolves and the Victoria coven. A meeting between the Quileutes and Cullens is scheduled in the forest. During the gathering Jasper highlights and demonstrates some skills and points needed to excel in combat versus a newborn Vampire. Bella, worried Edward will be harmed asks him to stay away when the combat begins.

Bella and Edward camp in the hills as the preparations for battle are made. They are soon joined by Seth and Jacob who too wait for the coming war. They use Bella’s scent and lay a trap to lead the newborns into the hands of the conjoined vampire and werewolf force. Bella enters a state of hypothermia in the freezing cold mountain conditions. Jacob taunts Edward by climbing in the sleeping bag with his girl. Bella pretends to be asleep and eavesdrops on the conversation between the vampire Edward and werewolf Jacob. Jacob speaks of his feeling towards Bella and confesses to Edward. Upon hearing this, Edward realizes that Jacob can give Bella something he can’t and the two come to an agreement.

Later in morning once they have risen, Edward speaks to Bella about the engagement, Jacob, who was hiding and listening to their conversation lets out an angered and hurt howl of pain. He leaves quickly before they can catch him. Bella speaks with Edward and asks him to go and find Jacob. Upon their return Edward leaves to allow Jacob and Bella to speak. Jacob threatens to get himself killed by joining in the conflict, upon hearing her dismay and the fact she doesn’t want him to Jacob blackmails her into kissing him. He says kissing him will stop him joining the fight and getting killed. She agrees and they kiss, a kiss that beckons the realization that she has feelings for Jacob but she’s in love with Edward far more than he.

Edward returns, having overheard the entire conversation. He explains that he isn’t angry with her for having such deep feelings for Jacob. This is a fact that makes Bella even more miserable than she was before. Victoria and the newborns attack during the groups stay at the camp. The combined werewolf and vampire force completely decimate and slaughter the inexperienced newborn army. Victoria had this planned though; she skirts the fight and arrives at the camp to confront Edward and Bella alongside a newborn by the name of Riley. Edward pleads with Riley, trying to put him off the fight and explains the fact Victoria is using him. Victoria convinces Riley otherwise and he refuses to switch allegiance. A fight occurs in which Riley is killed by the Lupine form of Seth and Victoria is killed by Edward. Bella watches the entire episode, causing Edward to worry for her thoughts and feelings toward him after seeing him as the unstoppable killing machine he is. She is unperturbed and throws herself into his embrace. She loves him dearly and has concerns for his safety and wellbeing alone.

The war is over long before the Volturi arrive to tidy things up. Edward is reminded of his agreement to turn Bella into a vampire. Alice replies that they have set a date.

Bella goes to visit Jacob who was injured in the fight. She reveals her final decision is Edward. Her love for him is far greater than anyone else alive and she hasn’t the will or desire to live another day without him around.

Bella later returns home from La Push. She speaks with Edward who announces that they don’t have to marry, that Bella can have her way without completing her wishes. Bella states that she wishes to do things Edwards’s way, and they agree to continue with the wedding first. He puts the ring on her finger and they both agree to reveal the great news to Charlie.

Born as Claire Farron (Éclair Farron) she began using her code name after the death of her parents in an attempt to emotionally reinforce herself so that she could protect Serah, but that only caused tensions to rise between them. Lightning is very independent and determined, but she is also shown to have a compassionate side. Lightning wields a gunblade called the Blaze Edge which is a combination of a gun and a sword, one collapsing into the other. She can also manipulate gravity with a device on her thumb (though due to Sazh's interference, this device becomes damaged). In battle, Lightning is very agile and uses acrobatic moves. He stands at a tall 200 cm (6'5"), and has light blonde hair under a black bandanna. He was nicknamed "Mr. 33 cm" by the development team of Final Fantasy XIII in reference to his shoe size. He uses his fists to take down opponents, focusing on brute force compared to the fast and agile Lightning. Snow bears the mark of the l'Cie on his lower left arm, through which he has the power to summon the twin Shiva Sister Eidolons, Nix and Stiria , who can combine into a motorcycle form for Snow to ride while toting a large gun. Xbox Live

Xbox Live Dashboard

When the Xbox 360 was released, Microsoft's online gaming service Xbox Live was shut down for 24 hours and underwent a major upgrade, adding a basic non-subscription service (Silver) to its already established premium subscription-based service (which was renamed Gold). Xbox Live Silver is free of charge and is included with all SKUs of the console. It allows users to create a user profile, join on message boards, and access Microsoft's Xbox Live Arcade and Marketplace and talk to other members. A Live Silver account does not generally support multiplayer gaming; however, some games that have rather limited online functions already, (such as Viva Piñata) or games that feature their own subscription service (e.g. EA Sports games) can be played with a Silver account. Xbox Live also supports voice and video communication, the latter a feature possible with the Xbox Live Vision; an add-on USB web camera designed specifically for the Xbox 360.The Gamercard of a player with a Silver account has a silver trim, rather than gold.

Xbox Live Gold includes the same features as Silver and includes integrated online game playing capabilities outside of third-party subscriptions. Microsoft has allowed previous Xbox Live subscribers to maintain their profile information, friends list, and games history when they make the transition to Xbox Live Gold. To transfer an Xbox Live account to the new system, users need to link a Windows Live ID to their gamertag on Xbox.com. When users add an Xbox Live enabled profile to their console, they are required to provide the console with their passport account information and the last four digits of their credit card number, which is used for verification purposes and billing. An Xbox Live Gold account has an annual cost of US$49.99, C$59.99, NZ$90.00, GB£39.99, or 59.99. As of May 14, 2008, Xbox Live has over 20 million subscribers.

Large Volume Liposuction, Safety and Indications

Large volume liposuction can be performed safely if certain guidelines are followed. Properly selected patients who have a good understanding of the expectations and limitations of the procedure tend to be very satisfied with the results. Importantly, physicians who are considering performing large volume liposuction must understand the physiology and differences from smaller volume liposuction. Applying the same standards and treatment parameters as with small volume liposuction may result in death and disaster.

The 5 pillars of safety must be followed at all times. The surgeon first must be well trained in smaller volume liposuction and have a thorough understanding of the physiologic implications of infusing and suctioning large amounts of fluids from the body. Cases should be scheduled and performed such that incrementally larger volumes of fat are removed as the physician gains experience. The anesthesiologist, present on every case, likewise must have a complete understanding of the procedure and be well trained to handle preoperative, perioperative, or postoperative problems. The facility where the procedure is performed should be accredited, properly equipped, and have experience with large volume liposuction procedures. Overnight care facilities must be available with registered nursing care and the appropriate monitoring equipment. Support staff at every level of the procedure, from the preoperative phase through recovery, must be competent and experienced.

Finally, the patient must be healthy and appropriately selected. Motivation, goals, and expectations must agree with what is clinically possible. Patients should be psychologically stable with good diet and exercise habits or evidence of motivation toward these habits.

In addition to these basic pillars, several other issues are worth repeating to ensure a safe outcome. Frequent communication between all members of the surgical team is critical. The tumescent technique of injecting very large volumes of wetting solution must be avoided in large volume liposuction, as this invariably leads to fluid overload and its associated problems. Instead, the superwet technique of fluid infiltration, in which volume infused is roughly equal to total volume removed (1:1 ratio), should be practiced. With this in mind, remember that enough wetting solution with epinephrine should be infused to maintain an essentially bloodless aspirate. If the aspirate becomes excessively bloody, reevaluate the procedure. In such cases, either the procedure should be terminated or more wetting solution should be infused for added hemostatic effect from the epinephrine.

Compressive postoperative garments always are worn to minimize postoperative bleeding, swelling, and third spacing of fluid. Finally, attention must be paid to maintaining the patient's core body temperature using heating blankets on the table, minimizing body exposure, using Bair Hugger-type warming blankets, and using warmed wetting solution.

Ultimately, the long-term results following large volume liposuction depend on the preoperative condition of the patient's skin, the patient's overall health and expectations, and the ability of the patient to maintain a healthy weight and lifestyle postoperatively. Whenever in doubt, consider staging procedures in terms of multiple liposuction procedures or combining the liposuction with other procedures.

Collagen and Other Injectable Fillers

Over the past 30 years, the methods available for the removal or improvement of acne scars and for the correction of wrinkle lines increased exponentially with the advent of new skin filler substances, improved techniques for elevating existing scars, and technology for abrading and resurfacing facial contours. Seemingly every month, a new and improved filling agent or laser, which will be the best of all available methods, becomes available. In the midst of these technological breakthroughs, the basic mechanisms remain the same. Three categories of techniques are presently available to improve acne scars: (1) scar removal and revision; (2) filling depressed scars; and (3) contouring the surface of scars.

Before and after images are below.

History of the Procedure

The goal for research and development in this field over the past 50 years has been the synthesis of the ideal filler substance for skin and soft-tissue augmentation. The ideal filling material has the following qualities:

  • Physiologic - Incorporates itself with the body's tissues
  • Simple procedure - Injectable
  • Risk free - No complications or adverse effects
  • Permanent - Does not degrade with time

This ideal has not been met but it has been the goal in creating all presently available materials.

Physiologic filling materials such as dermis, fascia, and fat have been used as autologous implants for many years, with specific surgical techniques used for each. These remain in use today but on the whole are cumbersome and complicated surgical operations with associated adverse effects and complications. Injectable filling materials included cutting oils, paraffin, and silicone, each with its own adverse effects and complications.

The first Food and Drug Administration (FDA)-approved injectable filling agent, bovine collagen, was approved in 1982 and remains the most commonly used product for skin and soft-tissue augmentation. In fact, Zyderm remains the criterion standard against which all new fillers are compared.

Thirty years ago, one either excised a scar or dermabraded the surface. Combinations of these two methods were beginning along with punch grafts and the use of autodermal grafts. The basic workup and evaluation of these scars remain essentially the same because these choices are available for the types of scars the clinician evaluates for improvement. This review discusses skin-filling agents available for scar correction.

Problem

Distensible cutaneous scars are best elevated with filling material. A distensible scar is defined as one that elevates to the surface when tension is placed on either side. This simple test allows the clinician to determine whether a particular scar is likely to respond to filling material. Fibrotic ice pick and fixed scars cannot be elevated because the scar tissue extends through to the subcutaneous tissue plane. Placing filling material in the dermis only elevates the surrounding skin and produces a donut effect, making the scar appear worse.

The ideal skin-filling agent is safe, physiologic, simple, and permanent. No known substance meets all of these criteria. Each of the available substances has adverse effects that detract from these properties, but all are the result of striving to perfect their qualities to fulfill these requirements. The list of injectable filling materials increases each month, with both synthetic and natural substances added. Other implant materials may be placed surgically.

Indications

Indications for collagen injection are acne scars, traumatic scars, photoaging, wrinkles, and gravitational rhytides.

Contraindications

Contraindications to this procedure include allergy to bovine collagen, allergy to lidocaine, pregnancy, or a collagen vascular disease such as dermatomyositis or polymyositis.
